EXTERNAL FACILITIES USE REQUEST

No requests will be accepted within 10 days of an event. Please plan accordingly.

All events require a $50 use fee. This should be sent within 3 days of the application and checks made payable to CRHS should be mailed to the attention of Debbie Brown at 625 Breakneck Rd., Mullica Hill, NJ 08062. Applications will be evaluated for usage based on the board policy for facility usage (click here for facility usage policy)

All required documentation (proof of non-profit status, Certificate of Insurance naming CRHSD as an additional insured under your GL policy, and team roster (including full name and address) must be received in our office within 3 days of the application and 10 days before your requested event date or the request will be canceled. Documentation can be sent via fax (856-223-2749) or email (brownde@clearviewregional.edu) or dropped off at the high school main office.

For fall usage, all requests must be received in our office no later than August 15 each year.

For winter gym usage, all requests must be received in our office no later than October 15 each year.

For spring field requests, all requests must be received in our office no later than March 15 each year.

Applications will not be processed until all necessary information is received which MUST be at least 10 days before the requested event date. NO EXCEPTIONS. $50 Usage fee must be received within three days of your application.

For recreational groups, you must confirm compliance with the district concussion policy 2431.4 linked below in the form.
