Only hard copies of the physical and health history forms are accepted.
DO NOT email the physical/Medical Eligibility form
or health history form.
Uploading it to your registration portal does not deliver to us. That is only seen by you.
If you miss the deadline to turn in your paperwork. We cannot guarantee clearance for the first day of practice. This is a firm deadline.

Fall Sports-Registration: June 9 through August 1, 2025
MS registration open June 9 through August 24.
Football & Girls Tennis begin 8/11/25
All other fall sports begin 8/18/25
Winter Sports-Registration: October 3 through October 31, 2025
Swimming/Diving begin 11/24/25
Basketball/Wrestling tryouts 11/24-11/26/25
All other winter sports begin 12/1/25
MS basketball tryouts week of 11/10/25
Spring Sports-Registration: January 28, 2026 through February 27, 2026
